Requirements

You may obtain a physical exam and Code of Conduct form from the office or your coach. Students will not be allowed to participate in practices or games without proof of a physical examination completed by a doctor, proof of insurance, a signed code of conduct, and a paid athletic fee. ImPact test or other concussion baseline test is recommended.
